Free Support for Eligible Patients

At Optima Performance & Health we provide free lifestyle, wellbeing and weight management support for eligible individuals through commissioned programmes and partnerships with public health, local authorities, schools and community services.

Our evidence-based programmes are designed to help individuals improve their health, manage their weight, reduce their risk of long-term conditions and develop sustainable healthy habits that last.

Whether you are looking to become more active, improve your nutrition, better manage a health condition or make positive changes to your overall wellbeing, our team is here to support you every step of the way.

Not Eligible for Funded Support?

If you are not eligible for a funded programme, have not been referred through a local service or funded support is not currently available in your area you can still access support from Optima Performance & Health privately.

Our self-funded services follow the same structured, evidence-based approach as our commissioned programmes giving individuals access to the same high-quality lifestyle, wellbeing and weight management support. This includes structured programme content, behaviour change support and one-to-one coaching tailored to individual goals and needs.

Our Services

Tier 2 Weight Management Programme

A structured lifestyle intervention programme designed to support individuals who are overweight or living with obesity, pre-diabetes, type 2 diabetes, hypertension or other lifestyle-related health concerns, including non-medical conditions that may benefit from improved nutrition, physical activity, behaviour change and healthier lifestyle choices.

Individuals receive:

  • Weekly education and behaviour change sessions
  • Personalised goal setting and action plans
  • Nutrition, Physical Activity, Sleep and Wellbeing support
  • Progress monitoring and outcome tracking
  • Ongoing coaching and accountability
  • Community-based or virtual delivery options

Tier 3 Behaviour Change & Weight Management Support

Individuals accessing Tier 3 support will receive all elements of the Tier 2 Healthy Lifestyle & Weight Management Service outlined above, with an enhanced and more intensive level of personalised support based on their individual needs.

This level of support is designed for individuals requiring additional help to achieve and sustain meaningful lifestyle and behaviour change, including those:

  • Living with complex obesity
  • Receiving  weight management medications such as GLP-1 therapies
  • Requiring additional behavioural support alongside clinical care
  • Transitioning into or out of specialist weight management services

Our Tier 3 support focuses on long-term lifestyle change, adherence, relapse prevention and improving overall health outcomes.

Children & Young People Programmes

We provide age-appropriate healthy lifestyle and behaviour change programmes for children and young people aged 0–19, extending up to age 25 for young people with SEND.

Our family-centred approach helps children and young people:

  • Develop healthy habits
  • Improve physical activity levels
  • Build confidence and self-esteem
  • Improve nutrition knowledge
  • Support healthy growth and development
  • Enhance emotional wellbeing

One-to-One Coaching

For individuals who prefer personalised support, we offer one-to-one coaching sessions tailored to their specific goals and circumstances.

Community-Based & Digital Support
